US Club Medical Release FormList Item 3
For U8-U12 Academy (all locations), U13-U19 NPL & SCCL (all locations) and U10-U19 Rec teams (Cartersville, Paulding, Marietta and Kennesaw) When completed by the parent/guardian of each player, this contains essential medical information and who to contact in the event of a player injury. This form also protects you as the coach from any financial obligations should you need to call for medical assistance for a player if the parent/guardian isn't present.

Weather Policy
The purpose of this policy is to educate our coaches, athletes, referees, and parents in strategies to ensure that all of our activities are conducted in a manner that best ensures the safety of our participants, and provides a set of guidelines for program directors and coaches to follow when determining the appropriateness of hosting a practice or scrimmage game.
